    Era Living is seeking a full-time MDS Coordinator Nurse (RN) to become the newest member of our Nursing team located in our Ida Culver House Broadview Community, Seattle, WA.

    Primary Responsibilities:

    The MDS Coordinator Nurse completes Medical Data Sets (MDS) for residents at the Health Care Center. This includes writing Care Assessment Areas (CAA) summaries upon completion of MDS, completing initial care plans and updaing care plans with any changes indentified in the MDS/CAA summary and/or observations made. This person will coordinate with nursing and inter-disciplinary teams to ensure timely completion of the assessment, run the restorative nursing program, and managing the restorative team. 

     

    Knowledge, Skills, and Abilities:

    • WA State RN, Resident Assessment Coordinator-Certified (RAC-CT)
    • Knowledge of and experience with the Resident Assessment Instrument (RAI) process, Medicare program and care management.
    • Excellent geriatric assessment skills and prior experience in a Skilled Nursing Facility.
    • Excellent communication and interpersonal skills when interacting with residents, family members, visitors and co-workers.
    • Ability to coordinate the MDS with nursing and inter-disciplinary teams to ensure timely completion.
    • Knowledgeable with managed care insurance process.
    • Knowledgeable with running a restorative nursing program.
    • Ability to pass DSHS and State background check, required.
